Duties/Skills
Implement and maintain a person-centred care model.
Oversee care plans and ensure services meet participant needs and regulatory requirements.
Maintain accurate records and documentation in the Client Management System.
Collaborate with clinical teams and stakeholders to enhance participant wellbeing.
Conduct home visits and manage risk strategies.
Qualifications or experience in Aged Care Case Management.
Strong knowledge of community-based aged care and support services.
Ability to work autonomously and as part of a team.
Excellent organisational skills and proficiency in Microsoft 365 and CRM systems.
Current police check, WWVP with NDIS endorsement, and driver’s license.
